Army identifies systemic failures behind DC midair collision and calls for safety fixes

The U.S. Army has identified numerous systemic breakdowns and failures as the underlying causes of a midair collision last year between an American Airlines jet and a Black Hawk helicopter near Washington, D.C.

In response to the findings, the military has called for a series of safety fixes and recommended numerous preventative steps the military should take to avoid similar tragedies in the future.
